A Mazda 3 and Toyota Landcruiser collided on Mylestom Drive about 7am on Saturday 1 August.
The vehicles, which were travelling in opposite directions, collided head-on after the Landcruiser allegedly crossed to the incorrect side of the road.
The impact caused the Mazda to travel down an embankment before coming to rest in the Bellinger River.
The driver, a 34-year-old woman, was taken to Coffs Harbour Hospital with minor injuries.
The male driver of the Landcruiser failed to stop after the crash; however, stopped the vehicle about 500 metres away and ran from the scene.
The man had put the vehicle in reverse before running, causing it to roll down a hill and roll multiple times.
Police from Coffs/Clarence Local Area Command are investigating the incident and attempting to locate the man involved.
Inquiries are continuing.
